In the insurance FAQ, I have written an article on "group rate" insurance policies. These are very low cost policies available within superannuation plans, and while they lack some of the bells and whistles of the more expensive policies sold by insurance agents, they do provide reasonable cover at a very reasonable price. You probably haven't heard of them because these policies pay a very low commission compared to regular policies.
